• Cyclic flexural loading was applied to SLM-built Ti6Al4V porous constructs.
• Flexural fatigue strength depended primarily on strut thickness and porosity.
• Numerical models predicted flexural fatigue strength with average deviation ≈ 10%.
• Fatigue cracks appeared to initiate at the surface of strut junctions.
• Simulated bone ingrowth resulted in greater flexural fatigue strength.
